Provisional Diagnosis - ?Polymyalgia Rheumatica TRAPS HIDS
Final Diagnosis - Tumour Necrosis Factor Receptor Associated Periodic Syndrome(TRAPS)
History and Findings - Polyamyalgia,Polyarthralgia,neck pain recurrent every 2 weeks lasting for nearly 4-5 days for last 2 1/2 yrs associated with fever.
Investigation - High ESR/CRP/CPK/LFT/RFT/TFT normal,RA,ASO,ANA neg.Serum p.EP no Mband,Thyroid antibodies normal.MP MF Neg,Immunoglobulins normal P.smear normal,Urine PBG neg,HBSAg,anti HCV & HIV Neg,ECG & CXR normal.Serum IgD normal.
Treatment - Colchicine (Goutnil)/Steroids(Tappering)/Proton Pump Inhibitor(PPIs)
Follow Up - 26/09/11-Pt asymptomatic
